We offer various client services for eligible clients to fill gaps in care not covered by other resources. If you are living with HIV/AIDS, reside in Kern County, and do not have adequate access to public or private Healthcare resources, we can help you. We have helped many HIV+ patients navigate available resources to have successful health outcomes. Maybe you currently have no other source of healthcare or have Medicaid or private insurance, and your care needs are not being met? That’s where we come in.
Based on an individual’s assessment, medical case managers may arrange, coordinate, monitor, evaluate, and advocate for a variety of services to meet the clients’ needs. Enhancing client self-care, independence, and self-determination are some of the goals of this program. Clients are involved and supported in the aspects of their care, including problem-solving functions to the maximum extent possible.
Proactive, coordinated efforts by our integrated HIV medical and case management team provide our clients with strong support in navigating and addressing healthcare needs. Ask how our case managers can assist you in obtaining optimum wellness, as well as making the best use of available resources.
Services
Our HIV and Related Medical Services provide compassionate, comprehensive care to individuals at all stages of their HIV journey. For those newly diagnosed, we offer expert guidance and tailored treatment plans, including antiretroviral therapy to manage chronic HIV infection and reduce complications. Our team is skilled in identifying and managing opportunistic infections, ensuring patients receive the specialized care they need.
Prevention is a cornerstone of our approach, with services like Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP) available to reduce the risk of HIV transmission. We also provide risk reduction counseling and education on safer sexual practices to empower patients with knowledge and tools for prevention.
Our care extends to managing HIV-associated comorbidities, including neurocognitive disorders (HAND), cardiovascular issues, bone health concerns, and liver disease linked to medication. For women with HIV, we offer family planning and contraceptive counseling, supporting reproductive health and personal choices. Through all aspects of care, we prioritize education, prevention, and support to improve health outcomes and quality of life.
Services are contingent on funding availability and include but are not limited to, Medical/Non-Medical Case Management, such as:

- ADAP medication support and other medication assistance

- Partner/HIV prevention services/PrEP Navigation and medication assistance

In a comfortable, caring setting, a Case Manager will meet with you for an assessment that will help determine how you can obtain access to HIV/AIDS healthcare and related supportive services and community resources. We also offer non-medical case management services extending to eligible clients’ families and social support systems.
