Filing Complaints

If you believe your privacy rights have been violated, you can file a complaint with Clinica Sierra Vista’s Chief Compliance Officer. You may send a written complaint to the U.S. Department of Health and Human Services, Office of Civil Rights. Clinica Sierra Vista’s Chief Compliance Officer can provide you with the appropriate address upon request.
