Clinica Sierra Vista has upgraded MyChart to better serve you. As part of this transformation, our new and improved patient portal is designed to give you easier, more secure access to your health information—all in one place.
Whether you’re managing appointments, viewing lab results, or messaging your care team, MyChart is your trusted companion for staying informed and connected. MyChart is available in multiple languages, including Spanish.
Please note: MyChart should not be used for emergencies. For urgent medical needs, please call 911 or go to your nearest emergency room.

Important Information for MyChart Accounts Created Before June 2, 2025
If you had a MyChart account before our system upgrade on June 2, 2025, please review the following updates to ensure uninterrupted access to your care:
- You will continue using your existing MyChart username and password. A small number of MyChart users received a new username to meet updated system requirements. If this applies to you, you’ll receive an email with your new login information. ➜ Click here to learn more.
- Proxy access must be re-established if you manage care for a child or loved one.
- Payment information must be re-entered to use billing, auto-pay, and paperless statements in the new system.
- For bills related to visits BEFORE June 2, 2025, please continue to use our legacy billing portal: ➜ Pay Older Bills
- For bills related to visits ON or AFTER June 2, 2025, please use the new MyChart portal: ➜ Pay New Bills
- Existing users were notified of the transition by email and/or text message, depending on the contact information we have on file.

When can I see my test results in MyChart?
You can view your lab and test results in MyChart as soon as they are available. Your provider receives the results at the same time, so you may see them before your provider has reviewed them. For common lab tests, we provide reference information to help you better understand your results. If any concerns arise, your care team will follow up with you directly.
What if some of my health information in MyChart is incorrect?
MyChart is a reflection of your electronic medical record at Clinica Sierra Vista. If something doesn’t look right, please let your provider know during your next visit. You can also update your personal and contact information directly within your MyChart account.
Is there a fee to use MyChart?
No. MyChart is a free service offered to all Clinica Sierra Vista patients.
How do I sign up for MyChart?
Signing up is quick and easy:
- During a Visit: Ask for an activation code at the front desk.
- Online: See the section above and click “Sign up with your code” or “Enroll in Self-Sign Up.”
My activation code doesn’t work. What should I do?
Activation codes expire for your protection. If your code has expired, you can still self-enroll online. For help, call our MyChart Support Line at (833) 678-2781.
I forgot my MyChart password. What should I do?
Click “Forgot Password” on the MyChart login page to reset your password securely.
Can I link my Clinica Sierra Vista MyChart account to other providers?
Yes! To link accounts:
- Log in to your Clinica Sierra Vista MyChart
- Click Menu
- Search for “Link My Accounts”
- Follow the prompts to connect with other MyChart-enabled healthcare organizations
How do I manage my MyChart notifications?
You can choose how you receive updates—text, email, or phone—by adjusting your notification settings in your MyChart account. These messages will never include personal health information.
