MyChart for My Family – Granting Proxy Access
At Clinica Sierra Vista, we know healthcare is a team effort. That’s why MyChart makes it easy for you to help manage the care of a child, parent, or loved one by offering Proxy Access—a secure way to view and assist with their medical information online.
Depending on the level of access granted, proxies can see upcoming appointments, test results, messages, and health reminders, and even help with billing tasks.

For Parents or Guardians: Accessing a Child’s MyChart

Ages 0–11:
Parents or legal guardians are granted full access to their child’s MyChart.
Ages 12–17:
Due to California privacy laws, parental access is automatically reduced to limited access. Teens can adjust access permissions in MyChart. If the teen consents, a provider or front desk staff can assist in re-establishing proxy access.
Age 18 and older:
Parental access ends automatically. Adult patients can re-grant access by sending a proxy invitation through MyChart or requesting it in person at their provider’s office.
For Adult Family Members or Caregivers: Accessing Another Adult’s MyChart
With Patient Consent
Adult patients can invite a trusted family member or caregiver to access their MyChart by submitting a Proxy Access Authorization through MyChart or by requesting assistance from their care team.
For Legal Guardians or Power of Attorney
If the patient is unable to grant access, a legal representative may request proxy access by submitting documentation such as a court order or durable power of attorney for health care (DPOA), along with a completed authorization form.
How to Set Up Proxy Access
Step 1:
Both the patient and the representative must have a Clinica Sierra Vista MyChart account.
If you’ve received care at Clinica Sierra Vista, sign up for MyChart in one of two ways:
Online: Visit our MyChart page to sign up through our secure self-enrollment process.
In Person: Ask your provider or front desk staff for an activation code during your next visit.
Step 2:
Send or Accept a Proxy Invite
Once both accounts are active, the patient can send a secure proxy invitation through MyChart.

- If the patient cannot send a proxy request, a representative may request assistance from the patient’s care provider or front desk staff.

What is proxy access?
Proxy access allows a parent, guardian, or trusted individual to securely view and manage a loved one’s health information through their own MyChart account. It’s a simple way to stay connected to your family’s care, whether you’re helping with appointments, test results, or billing.
How does proxy access work for children?
Ages 0–11: Parents or legal guardians are granted full access to the child’s MyChart record.

Can I access another adult’s MyChart account?
Yes, with the patient’s consent. The adult patient must complete a MyChart Proxy Access Authorization Form, which includes their approval to share access. You can also request help at the front desk or with your care provider.
What if I’m a legal guardian or hold medical power of attorney?
If the patient cannot consent, the representative must submit a completed Proxy Access Authorization Form along with valid identification and supporting legal documents, such as a court order or Durable Power of Attorney for Health Care (DPOA).
Can I request proxy access for more than one person?
Yes. You may request proxy access for multiple qualifying individuals. Each request must go through the same process and be authorized separately.
Can I ask questions about a family member using my own MyChart account?
No. To protect privacy and ensure accurate records, all messages sent through MyChart are stored in your personal health record. Communicating on behalf of someone else without proxy access could misdirect medical information and impact care.
Can my spouse and I share one MyChart account?
No. Proxy access does not make you responsible for paying the bill. The guarantor—the person listed on the billing statement, is responsible. However, as a proxy, you’ll be able to view and help manage bills through MyChart.
What billing features can I access as a proxy?
With proxy access, you can:
- View billing statements and payment history
- Make payments and set up payment plans
- Access cost estimates and paperless billing options
To manage a loved one’s billing:
- Choose the name of the person you represent to view and manage their information.
- Log into your MyChart account.
- Click on the Billing tab.
