Jason West

Jason West, MPH, serves as Chief of PACE at Clinica Sierra Vista, where he leads the design and launch of PACE clinical operations, quality infrastructure, regulatory readiness, and community engagement. He is responsible for building a deeply interdisciplinary team and establishing a participant-centered model that balances clinical sophistication with what matters most to older adults and their families.
Jason’s journey into healthcare leadership began early through volunteer service in skilled nursing facilities, experiences that shaped his lifelong commitment to dignity-centered elder care. His career has included collaborative work supporting unhoused patients discharged from hospitals, leadership in launching Hope Respite Care in Merced County, and establishing Central Valley PACE serving Merced, Stanislaus, and San Joaquin counties. He has also contributed his expertise through board service with the Merced County Rescue Mission and leadership roles within state and national PACE associations.
Grounded in systems thinking and data-informed strategy, Jason integrates analytics with human-centered leadership to align interdisciplinary teams and continuously improve care delivery. His work at Clinica Sierra Vista reflects a deep connection to the organization’s mission, expanding access, strengthening community trust, and building care systems rooted in compassion, integration, and dignity.
