care for all patients. As part of our commitment to accessibility, our
Health Insurance Assistance Program provides additional eligibility screening
and application assistance for all programs that patients may qualify
for. We offer a sliding fee discount based on family size and income.
Patients who qualify for the Sliding Fee will receive the Patient Assurance Cards.
