Reaching Families Where They Are
Clinica Sierra Vista’s (CSV) Mobile Unit Program continued to expand across Kern and Fresno Counties, delivering quality care to the communities we serve. With support from the Kern Health Systems Quality Grant, we launched our third mobile clinic, the Steve Schilling Mobile Unit, to reach families facing barriers such as transportation challenges, homelessness, or lack of insurance. Operating three full days a week, the program established regular clinics at two recovery centers and added a full-day clinic at Poverello House, ensuring high-need communities have consistent access to care where they live and seek support.
Beyond medical care, these units act as trusted access points, helping patients connect to mental health services, ECM, and ID/voucher programs. Each mobile clinic is staffed by a dedicated team of three Medical Assistants, one LVN, and a lead provider, delivering every visit with dignity, quality, and compassion.

Our mobile care team offers a variety of services to support your well-being, including:
- Primary care
- Well child exams
- Chronic disease management
- Women’s health services (Fresno County only)
- Addiction medicine
- HIV prevention and treatment
- Mental health medication refills
- Preventive vaccines

How to Access Mobile Clinic Services
Mobile clinic services may be available at scheduled community locations, partner sites, outreach events, shelters, recovery centers, schools, or other community-based locations.
Some mobile clinic visits may be available by appointment, while others may be offered on a walk-up basis, depending on the location, service, and event schedule.

Appointments and Registration
Before a visit to the mobile clinic, patients may need to fill out a short registration form. This process helps our care team understand your health needs, verify your information, and ensure you receive the appropriate care.
Please bring the following, if available:
- Photo ID
- Insurance or Medi-Cal card, if applicable
- Immunization record or vaccine card
- List of current medications
- Any paperwork provided by a referring organization or community partner

If you do not have one or more of these items, please still contact us or speak with our mobile clinic team. We will do our best to help you access care.
Immunization Records
If you or your child are visiting the mobile clinic for vaccines, please bring an immunization card or vaccine record if you have one. This helps our care team review which vaccines have already been received and determine which vaccines may be needed.
If you do not have a copy of your immunization record, our team may still be able to help. We can review available information and guide you on the next steps.

Cost of Services
Clinica Sierra Vista is a Federally Qualified Health Center. We are committed to caring for our community and helping patients access care regardless of their ability to pay.
The cost of services may depend on your insurance coverage, program eligibility, household income, and the type of service provided. For patients who are uninsured or underinsured, CSV may offer assistance through programs such as Medi-Cal, Covered California, and the Sliding Fee Discount Program.

The mobile clinic team can help connect patients to ongoing care with Clinica Sierra Vista providers.
If you need follow-up care, specialty care, lab services, behavioral health support, dental care, pharmacy services, or a regular primary care provider, our team can help refer you to the appropriate CSV health center or program.
